How to Submit an Extra Duty Pay Request Form
Important Information:
- Form Status: You can check the approval status of your form online using a custom link that will be emailed to you after you submit your form.
- Organized by Pay Period: Each form submitted will contain only dates worked within that pay period.
- Exceptions: The following will not use electronic forms. Please contact these departments for information about how to request Extra Duty payments:
